Requests for Information and Assessments for External Providers:
Gainsborough State School recognises the value of collaborating with families, medical professionals and other external providers to best support our young people. As part of the external provider's process of assessing need and supporting students, schools may be requested to provide information regarding a student's current presentation, progress, functioning and support needs.
Given that the information involved is often of a sensitive nature and can rely on specific administration protocols, Gainsborough State School adopts a best practice approach when responding to requests received from external providers. Requests for information must be made by parent/caregivers, in writing to the school Insight Committeethrough the completion of the below forms.

Forms can be returned to the administration office or alternatively, by email to the Insight Committee insight@gainsboroughss.eq.edu.au
Please note: Schools are not expected to undertake any new assessments or create new documents to support NDIS applications or reviews.
External Providers Onsite:
As our school continues to grow, we are committed to providing the best possible learning environment for all students. Due to the increasing number of enrolments and the limited availability of suitable spaces, we are unable to accommodate requests for external providers to work onsite with individual students during school hours.
We understand that some families may wish to engage external professionals to support their child's development. While these services cannot be provided on school grounds, we encourage parents to schedule appointments outside of school hours to ensure minimal disruption to their child's education.
